Mentee Application Form – Fall 2025 Session

By filling out this form, Mentees agree to abide by the Participant Expectations, the CWRA Code of Ethics, and the CWRA Anti-Harassment Policy during their participation in the program. Filling out this form is not a guarantee that the applicant will be able to participate in the Fall 2025 session.

Thank you for your interest in the CWRA mentorship program! Submitting this form will allow us to prepare for our sessions and ensure that we have mentors who can meet the needs of our participants. Please note the following: 1. This program is a national program, and all the events for the program, including meetings with assigned mentors, will be held virtually. All participants will need to have access to a computer with an internet connection, and are expected to attend each meeting. 2. To participate as a mentee in this program, you need to be a member of CWRA. 3. Mentee participation in this program is limited and based on mentor availability. Please apply early! 5.
